פטיש לכאב (באנגלית: Algolagnia) הוא משיכה מינית חזקה (פטיש) לכאב, בעיקר באזורים הארוגניים, דוגמת הפין, הפות, פי הטבעת והפטמות. מקור המונח הלועזי מיוונית, בה algos (ביוונית άλγος) היא כאב ו-lagnia (ביוונית λαγνεία) היא תשוקה.
מחקרים אשר נערכו בקרב אנשים עם משיכה חזקה זו הצביעו על הבדלים בדרך פירוש קלטים עצביים הנובעים מכאב ואישרו למעשה את קיומה של המשיכה המינית החזקה.
